Please use the link below to access the Government of Ontario School and Child Care Screening Tool. The results of the screening will let you know if your child can attend daycare today. Once you have your results please complete and submit the form below to confirm your results.
If the results of your school screening advise that your child cannot attend care, please contact your daycare provider immediately to advise which of the three options you will be using before returning to care.
